If you don't accept my plan, what should I ask insurance to understand my Out of Network Benefits?

When inquiring about using your out-of-network benefits, consider asking your insurance company the following questions:

  1. What is my deductible for out-of-network services, and how much have I met so far?
  2. What percentage of the session fee will be reimbursed once the deductible is met?
  3. Is there a maximum number of sessions covered or an annual limit on out-of-network benefits?
  4. Do I need pre-authorization or a referral to use my out-of-network benefits for therapy?
  5. What documentation is required for reimbursement (e.g., superbill, diagnosis code)?
  6. How do I submit claims, and how long does the reimbursement process typically take?
  7. Are there any specific exclusions or limitations for mental health services in my plan?
